Cancellation Policy

All services require 24 hour cancellation and reschedule notice. In the event that 24 hours notice is not given you will be responsible for the cost of the service. In the event of a no-show for your scheduled appointment, you will be responsible for the cost of the service. In the event of late arrival for your scheduled appointment, your therapist may need to deduct the amount of time you were late from your service, you will be responsible for the full cost of the service.

Draping Policy

It is the policy of Anoka Massage and Pain Therapy that clients remain draped at all times. Clients will have a sheet and/or blanket draped over you at all times. Your therapist will adjust draping as he/she works to expose areas (never the genitalia) and then recover them and move on.

Your comfort level is very important to us and although it is customary to remove all clothing, you do not need to be nude underneath the draping, you can leave as much or as little clothing on as you like. Please do NOT remove the draping during your treatment, even if the room temperature is too warm. You can ask your therapist to adjust the room temperature, but the draping must remain on at all times.

Sexual Conduct Policy

WARNING: Due to the potential for misguided clients to make inappropriate sexual advances in this industry, we wish to prevent any embarrassing predicaments by making it clear to all clients in advance, that any solicitation for overt sexual acts, subtle hints or comments regarding “full release” or “happy endings” (even if made as a joke) will result in the immediate termination of the treatment. Please be advised that removing or attempting to remove the draping to expose genitalia or buttocks will be considered inappropriate conduct. Should you require techniques in those areas of the body,
please discuss your need with your therapist prior to the treatment as it is our strict policy to call the police, file a report and follow through with the filing of charges should any misconduct or requests for illegal acts occur. Should such an unfortunate incident occur, you will be required to pay the full amount of your treatment. Failure to pay for your treatment could result in further filing of charges.

Accessibility

Our main level facility meets handicap accessible guidelines including: 36″ doors, no stairs, large restrooms with support rails, and handicap accessible massage and acupuncture tables that lower down to 19″.  We also have therapists trained in providing therapies for disabled and special needs patients.

We value the dignity and worth of all persons regardless of age, race, culture, creed, sexual identity, gender, ability and/or health status.

AED & CPR

We have an Automatic Electric Defibrillator placed on site. As on ongoing effort to provide continued education and to be prepared for unexpected events, our staff is trained in the use of the AED and certified in CPR in the unlikely event of an emergency at our clinic.

Beyond Wellness Membership Program terms

Authorization: By purchasing this membership, you agree and authorize Anoka Massage & Pain Therapy to annually charge your credit card on file for the Beyond Wellness Membership fee. This annual fee will be made one year from your start date and you agree not to dispute Anoka Massage & Pain Therapy for this reoccurring fee. Furthermore, you agree to the terms and conditions set forth in this Agreement and understand that you will forfeit any benefits of membership to date if your account is terminated.

Termination of Membership: Anoka Massage & Pain Therapy requires a 30 day written notice prior to the anniversary month to cancel the scheduled membership renewal transaction. Email your notice, or send hand written notice to: 710 East River Road, Anoka, MN 55303.

Membership Fee: The membership fee is $50 for a single membership, $100 for a family, and $0 for seniors age 65 and older. Anoka Massage & Pain Therapy has the right to change the fee with 30 days written notice to its members. Therefore, you guarantee and warrant that you are the legal cardholder for the credit card on file and that you are legally authorized to enter into this annual charge agreement with Anoka Massage & Pain Therapy.  If requested, a pro-rated refund, either by the discounted amount of services or time in effect, may be issued.

Note: A single membership includes one adult and any children under the age of 18. A family membership is for two adults and any children under the age of 18. Please notify the office with the names and birth dates of children that you want added to your membership. See our website for current pricing and membership benefits.

Third Party Payer Agreement for Medical Massage (work comp, PIP, and auto insurance claims)

The responsibility for the cost for each massage therapy and or acupuncture session is the clients. I understand that my insurance company will be billed $80.00 per unit (15 minutes) for massage and $100.00 first unit, $75 second through fourth units for acupuncture. Whatever portion of the session not covered by a third party payer is the clients responsibility. I agree to pay the amount that the third party does not pay. If the third party notifies this office that benefits are exhausted you will be responsible for paying the unpaid balance of your invoice.
